Purnima Sreenivasan of Health Aim is an Aging Life Care Specialist who provides the following:

– Solutions for safety, early recognition of issues and prevention of issues with specific plans of care, such as housing, falls, monetary, nutrition etc.

– Help in easing difficulties and providing recommendations for life enjoyment, making it a win-win situation for all concerned in the care of the client
– Help in navigating and planning End of Life services according to the client’s specific death issues
– A commitment in making a true difference in someone’s life!

Purnima Sreenivasan is a compassionate, pleasant and kind professional whose motto is to provide the best care at each and every visit. She loves listening to her patients, families, conservators, and other legal advocates, understanding their problems and providing individual-based care. She loves her work and has been in private practice for over 16 years. She is a strong advocate for disease prevention and decreasing hospitalizations.

Purnima Sreenivasan’s goal is to ensure the highest quality of personal care. She held Board Certification from the American Board of Family Medicine and has completed a Master of Public Health in Community Health Education.

After graduating in MBBS (Bachelor of Medicine, Bachelor of Surgery) in the Top Five of her class, she moved to the United States to further her education. She completed St. Joseph’s Family Medicine program, received a Master of Public Health from San Jose State University and completed a Fellowship in Geriatric Medicine from University of Hawaii. She also served as the Medical Director for hospice, home health, and nursing facilities, and served on the Meals on Wheels Board of Directors.
